The purpose of this study is to analyze major causes of the surge in the medical expenditure using `cost of dying`, that is, medical expenses just before the death and overtreatment, and to identify the effect of enormous amount of medical expense before death on quality of life. This study used Korea Health Panel data from 2009 to 2013 to examine the effect of time to death and overtreatment on the medical care expenditure and the causes of overtreatment. In addition, we analyzed the determinants of the cost of dying spent at the end of life and confirmed their impact on the quality of life. As a result of the analysis, the medical expenditure was significantly increased as the death was imminent. The closer to death, the greater probability of overtreatment and overtreatment significantly increased the medical care expenditure and the cost of dying. However, the cost of dying at the end of life did not have significant effect on the quality of life. This suggest that there is no efficiency in cost of dying, which pointed out to be excessive, and it is necessary to reduce medical expense at the end of life and replace it with hospice and palliative care. This study is meaningful in the sense that it investigated the relationship between overtreatment and the medical expenditure as well as the cost of dying and examined the effect of the cost of dying on the quality of life.
